Bad breath: what if it came from your gums, not your stomach?

"It's my stomach." That is the explanation you hear most often when bad breath comes up. Yet it is wrong in the vast majority of cases: the reference literature in periodontology agrees on an order of magnitude of around 85 to 90% of bad breath being strictly oral in origin. And within that, a very large share comes neither from the tongue alone nor from what you have eaten, but from the edge of your gums.

Good news: that is precisely the easiest cause to put right, provided you aim at the right place.

Why do gums produce bad breath?

The smell does not come from the bacteria themselves, but from what they release. Below the gum margin — that gap of one to three millimetres between the gum and the tooth, called the sulcus — oxygen barely circulates. It is an ideal environment for anaerobic bacteria, the ones that thrive without air.

These bacteria feed on proteins: food debris, dead cells, and above all the proteins contained in blood and inflammatory fluid when the gum is irritated. As they break them down, they release volatile sulphur compounds, the well-known VSCs. Three molecules dominate:

  • Hydrogen sulphide — the characteristic rotten-egg smell.
  • Methyl mercaptan — a fermented-cabbage smell, the most stubborn of them; it is the one that dominates in breath of gum origin.
  • Dimethyl sulphide — rarer, often associated with a non-oral origin.

One detail that matters: methyl mercaptan is markedly more abundant when there is gum inflammation. In other words, the more the gum bleeds, the more it feeds the bacteria that smell bad. And the more those bacteria multiply, the more the gum becomes inflamed. It is a vicious circle, and it spins fast.

How do you know whether it comes from your gums or from your diet?

There is one simple and surprisingly reliable clue: how quickly the smell comes back after brushing.

The logic is mechanical. A strong-smelling food (garlic, onion) passes into the blood and is eliminated through the lungs within a few hours. A bacterial colony established under the gum, on the other hand, rebuilds itself in less than an hour after a surface brushing, because you never reached the zone where it lives.

The floss test: run a piece of floss between two molars at the back, where you almost never go. Pull it out and smell it. If the smell is frankly unpleasant, or if the floss comes out tinged with pink, you have just located the source. Repeat the test between two incisors: the difference in smell is often spectacular.

And what about the back of the tongue?

The back of the tongue is the other great reservoir of VSCs. Its surface is not smooth: it is covered with filiform papillae that form micro-cavities, and that whitish coating you see when you wake up is a bacterial mat in its own right. In many people, tongue and gums contribute to the problem together — treating one without the other only gets you halfway.

The technique that works:

  1. Use a tongue scraper rather than your toothbrush: it scrapes off instead of spreading around.
  2. Start from the back, as far back as your gag reflex will tolerate, and pull forwards.
  3. Rinse the tool between each pass. Three or four passes are enough.
  4. Do not scrub hard. A raw tongue does not smell any better, it is simply injured.

Do it once a day, preferably in the morning. It takes thirty seconds.

The routine that breaks the vicious circle

If the origin is in the gums, brushing harder or longer will change nothing: the brush does not go below the gum margin. What counts is where you clean.

  • Brushing angled at 45° towards the gum, soft bristles, two minutes, twice a day. The angle matters more than the pressure.
  • The tongue scraper, once a day.
  • Saliva. A dry mouth concentrates smells. Drink regularly; breathing through the mouth at night explains a great deal of morning breath.

FAQ

Does mouthwash solve the problem?

It masks it for a few hours. Scented mouthwashes cover the smell without touching the bacterial biofilm under the gum. Worse, alcohol-based ones dry the mouth out and can make the situation worse in the medium term. A mouthwash is an add-on, never a substitute for mechanical cleaning.

Why can't I smell my own bad breath?

It is the phenomenon of olfactory adaptation: your receptors, permanently exposed to the same molecules, stop signalling them. That is why the floss test is useful — it lets you smell the source concentrated, outside the usual flow.

My breath still smells after a scale and polish, is that normal?

For a few days, yes: the gum has to heal. Beyond two or three weeks with daily interdental hygiene, go back to your dentist. There may be deeper periodontal pockets, which call for root planing — a cleaning below the gum that no toothpaste can replace.

Is tobacco to blame?

Doubly so. It brings its own smelly compounds, but above all it reduces blood flow in the gums, which masks bleeding while letting inflammation progress. If the breath persists after a month of rigorous interdental routine, or if you notice regular bleeding, receding gums or loose teeth, make an appointment.
